In this webinar, staff of the Diabetes Resource Center in Providence, RI will discuss the key components in creating and evolving a hospital-affiliated diabetes program that addresses the needs of the uninsured, the underinsured and the homeless. Presenters will describe practices developed by the program to help people who are diabetic reduce the risk of diabetes related complications and hospitalizations. Through a combination of patient self management, staff assistance and integrated chronic care services the program has demonstrated positive clinical outcomes for its patients.

By the end of this webinar, you will be able to:
 
1. Describe resources available that enable uninsured or otherwise vulnerable patients to obtain medications, diabetic supplies and material support needed to work toward positive treatment outcomes.
 
2. Identify strategies to maximize internal and external resources to provide patients with nutritional, mental health and additional chronic care services critical to treating the comprehensive health needs of diabetic patients.
 
3. Identify educational and peer support opportunities to engage patients in taking a significant role in managing their own care.
