Software to track Medications Recommendations

I work in a Community Mental Health program and we are interested in starting up a PAP to assist our patients who are indigent and either have not applied for Medicaid/Medicare or at not eligible.  We do have a part-time pharmacist who will assist us with the packaging and labeling. 

What we are in need of is software that will track these medications.  Specifically we are looking for a good program that will allow us to track medications that come in from the pharmaceutical companies, keep tract of bottles that contain more than a 30 day supply so that the bottle is identified by the patient it is intended for so that monthly prescriptions can be delivered to the patient from their bottle. 

Can anyone recommend any software programs? General retail pharmacy software does not seem to meet the needs we have to be able to keep each bottle identified with the patient so that we are not using any approved PAP medications for any patient other than the one that qualified to receive it from the company.
